How to enter the survey:

1. To start, you should have a recent Sam’s Club cash register receipt at hand. When you open the survey site, select your preferred language to take the survey. Then read the terms and click on the “Continue” button.

2. On the next page, you need to provide the Club Number and TC number found from your receipt. Enter them and click “Continue”.

3. Next, you can start to answer the survey questions. For example, how satisfied were you with your visit to this store, how likely are you to shop again, how much does your household spend on groceries, and so on. The survey will take you approximately 10 minutes to complete.

4. Once you answer all questions, you need to provide your personal details and click on the “Submit” button. Then you’be entered into the customer survey sweepstakes automatically.
